Available here are the forms used for various functions within the parish. If you need a form not available here, sent an email to Webmaster@StJames-Greater.com letting me know. Please print and complete the needed form and drop it at the Parish Office.

Note that these forms are made available in PDF format and require a reader program on your computer to view and print. A free reader is available from Adobe® and can be obtain by using the link provided in the left panel.

ACH-Automated Clearing House	St James Parish Family offers our parishioners an opportunity to make their monthly donations using automated electronic payments. This program is available regardless of where you bank.
Request for Services	This is the form to request many of the parish services which are of interest. Included are Prayer List and Spiritual and Health Care Services, including hospital or home visit,
Facilities Request and Scheduler	This is the form to use to reserve rooms and facilities of the parish. Includes materials, setup and resources that will be needed.
Parish Registration	This is the registration form (Membership Application) required of all new parishioners. It is critical this form be completed before requesting any of the Sacraments for you or your family.
Scrip Order Form	This is the latest order form for scrip available from the St. James Parish Family Program.
